• The ICU is open seven days per week, 24 hours per day. 
ICU specializes in:

• Understanding complex pharmacology

Most common diagnoses: Patients admitted to the ICU are critically ill with diagnoses including sepsis, respiratory failure, traumatic brain injury, multi-system organ failure, stroke, gastrointestinal bleeding, complicated post-operative issues, status post cardiac arrest, complex vascular surgery, and multi-system trauma.    

Professional assets: 
• ICU patients require frequent assessment and intervention to maintain hemodynamic stability through complex treatments including aggressive titration of vasopressors, pulmonary artery monitoring, advanced neurological monitoring, mechanical ventilation, CPAP/BiPAP, proning, targeted temperature management, continuous renal replacement therapy, and continuous ECG/EEG monitoring    
• The team places the greatest value on patient-centered, evidence-based practice.

Salem Hospital’s 30 bed cardiovascular unit is situated on the 5th floor of the Critical Care Tower (Building A). CVCU provides care to patients 16 years-of-age and older, who have primary cardiac diagnoses, including heart failure, post-cardiac arrest, cardiothoracic (CT) surgery (CABG, Valve repair/replacement, VATS/thoracotomy), IABP therapy, Impella insertion, post PCI, pacemaker insertion, ablation, arrhythmia, STEMI/NSTEMI, EKOS. CVCU is a blended unit which includes 18 beds designated as cardiovascular intensive care (CVICU) and 12 beds as cardiovascular intermediate care (CVIMCU). The CVICU utilizes a cardiac universal bed (CUB) model for patient care in which immediate post-operative CT surgery patients are recovered, transitioned through the phases of care and discharged from the same room. CVCU patients often require frequent assessment and intervention to maintain hemodynamic stability through aggressive titration of vasopressors, complex treatments to prevent further deterioration, pulmonary artery monitoring. The CVCU care team is comprised of CT surgeons, cardiologists, PAs, intensivists, nurses, nursing assistants, critical care techs, respiratory therapists, pharmacists, physical therapists, dietitians, social workers, care managers, and chaplains. The team places the greatest value on compassionate bedside care, and evidence-based practice.

The CVCU, as a Beacon Silver Award recipient, functions using a model of shared governance. Key decisions are made in collaboration among unit leaders and staff. The CVCU staff is represented by the unit Special Practice Team (SPT) consisting of registered nurses, the leadership team (nurse manager and assistant nurse managers), critical care techs, and certified nursing assistants. The SPT serves as a venue to which staff members address concerns and/or questions regarding unit practice. Any issues brought to SPT are reviewed, discussed, and appropriate actions are taken. The unit promotes a positive and supportive work environment of collaboration among colleagues and leaders.

The CVCU nursing staff has the opportunity for advancement through its professional development pathway, which utilizes the Benner’s novice to expert nursing theory as a framework. From beginning intermediate level care through open heart recovery and care, identified as Levels 1 – 6, education and clinical orientation are provided. The hospital’s New Grad Residence Program provides first-time nurses with a blended learning process of computer-based education, precepted clinical training, and classroom experience sensitive to the local diverse and multicultural environment.

A 7-bed high-risk perinatal care unit which reflects our philosophy of family-centered care. Patient and family member learning is supported by our interdisciplinary service team members. Scope of service: Nursing care is provided for non-telemetry, non-PICU pediatrics (ages 17 and below), adult medical and surgical patients with 24- to 48-hour stay, and for pediatric patients requiring intermediate care, at the discretion of the provider and Pediatric Charge RN. Pediatrics also provides pediatric outpatient care for local clinics.

This unit provides pre-surgery screening and evaluation of, and communication with, each surgery patient. Nurses also provide discharge phone calls following outpatient surgeries and procedures. Physician documents as well as lab and test results are collected for early chart preparation. Nurses and anesthesiologists telephone each patient for verbal counseling and information sharing, leading to complete pre-surgical case reviews prior to patient admittance. An RN is scheduled three days a week at Hope Orthopedic to provide face to face pre-surgical patient preparation.

Scope of service: 
• The post-anesthesia nurse specializes in a wide variety of surgeries and surgical diagnoses that are found in both pediatric and adult populations. 
• Within the PACU, the clinical RN is expected to be proficient in airway management, invasive blood pressure monitoring and administering inotropic agents, evaluating for conditions such as sepsis, or any other physiologic change that could be elicited by general anesthesia.

Most common diagnoses: 
• Surgical procedures range from routine appendectomies and cholecystectomies to craniotomies, video-assisted thoracotomies, and complex vascular surgeries.

Professional assets: 
• Within in the PACU, a strong critical care background is a must. 
• The post anesthesia nurse must be able to function independently and in collaboration with other nursing staff and medical providers. 
• The PACU is fast-paced environment where the bedside nurse must be able to identify trends and intervene quickly.

Shift hours and unit-based resources:
• Shift lengths are traditionally 9 to 10 hours long with variable start times of 0800, 0900, 1000, 1100, or 1200. 
• Night shift staff work a traditional 12-hour shift with a start time of 1900. 
• Weekend call is required.
• Department resources consist of CNAs, float/resource nurses, charge nurse, assistant nurse manager, nurse manager, and AIC or Anesthesia in Charge.

Scope of service: Specializing in pre-surgery preparation as well as stage 2 recovery for patients being discharged to home after surgery.

• There is no common diagnosis.  
• As a team member you would prep and/or recover everything from an open heart to an esophogastroduodenoscopy and everything in between.   
   
Professional assets: 
• A busy unit with quick patient turnover, requiring employees who are self-motivated, have great attention to detail, and able to deliver timely competent nursing care. 
• Frequent skills include IV starts, nerve blocks, and solid assessment skills.

A 24 bed unit specializing in the physical rehabilitation of individuals with both surgical and traumatic orthopedic and neurologic conditions, strokes, and a wide variety of musculoskeletal conditions that impact an individual’s functionality and independence. The unit utilizes an interdisciplinary team model lead by 2 board certified physiatrists who are located on the unit 7 days per week. Our unit boasts a beautiful therapy garden for outdoor activities and a therapy apartment, both aimed at allowing patients the opportunity to practice skills they will need for a safe return to the community.

Division: Imaging RNs

Unit and location: First floor, building A

Specialty certification needed: BLS, ACLS, PALS or ENPC may be substituted for PALS. Telemetry monitoring and demonstrated proficiency in cardiac rhythm interpretation. Conscious sedation competency mandatory. Nurses should have critical care background. Optional certification in certified radiology nursing (CRN).

Size of unit, number of beds and treatment areas: Any imaging department room may serve as a procedure room. Rooms dedicated to procedural care include ultrasound 3, 4, and 5; CT rooms 1 and 2; and mammography imaging rooms 1 and 7. There is limited procedural capacity in staging bays 3, 4 and 5.

Scope of service: Specialize in providing ambulatory outpatient care as well as inpatient procedure based care. Patients are med-surg to intermediate care level patients. Procedures are minimally to moderately invasive needle based-procedures that may or may not require conscious sedation. Conscious sedation provided for noninvasive imaging for patients requiring such. Cardiac telemetry monitoring for patients requiring such and may be in department for an extended period of time. Cardiology care can include assisting with transesophageal echocardiograms (TEEs), cardioversions and assisting with dobutamine cardiac stress tests. RN staffing is typically 1:1.

Most common diagnoses: ESLD, cirrhosis, Hepatitis A/B/C, pulmonary disease, heart failure, renal disease, cancer, r/o cancer, cardiac arrhythmia, r/o MI, drain placement.

Professional assets: Imaging RNs work hand-in-hand with a variety of professionals including radiologists, interventional radiologists, physician assistants, ultrasound technologists, CT technologists, MRI technologists, NM technologists, X-ray technologists, mammography technologists and telemetry trained unit clerks. Imaging RNs also provide back-up care for cardiologists.

Shift hours and unit-based resources: Imaging RNs staff between the hours of 0700 and 1930, Monday thru Friday, to a core of 4 RNs. Weekend coverage is between the hours of 0800 and 1800. Weekend staffing is 1 RN only. There is no call for imaging RNs. Unit based resources include 2 adult code carts, 1 pediatric code cart, 3 omnicells, 10 MP5 monitors with a central monitor system, ceiling lift for bariatric patients located in zone 2 of MRI.

Collaborative resources: Imaging doctors, physician assistants, lead imaging technologists, and imaging managers. Imaging RNs can also call on house resources such as rapid response team, assigned house supervisor, patient transportation, and interpretative services.

The Cath Lab is located in Bldg A, 2nd Floor. The department supports a patient population primarily consisting of adults (both inpatients and outpatients) with cardiovascular disease or those in need of interventional procedures that require fluoroscopy. This includes inpatients and outpatients referred by primary care physicians, cardiologists, vascular surgeons, intensivists and nephrologists. Patient needs include continuous cardiac monitoring, procedural sedation, femoral arterial access management, and often times other interventions such as intubation and/or cardioversion or defibrillation. A multi-disciplinary team including radiologic technologists, registered nurses, and support staff assist and support patients and physicians throughout the intraprocedure process. The Cath Lab treats an average of 22 patients/day in 5 procedure rooms. Hours of operation are 0630-1800 Mon-Fri with an on-call staff evenings and weekends to manage emergency cases. The On-Call team has a designated 20 minute response time from time of page until they are dressed and in the procedure room preparing to care for the designated patient.

The Radial Cath Lab is an extension of Cath Lab services, located across campus from the main Cath Lab, in Bldg C, 3rd Floor. The Radial Cath Lab is focused exclusively on the outpatient population where catheterization is suited for the radial artery approach. This department supports freely mobile adults with cardiovascular disease referred by primary care physicians and cardiologists. Patient needs include cardiac diagnosis and intervention by fluoroscopy along with continuous cardiac monitoring, procedural sedation, and occasionally other interventions. A multi-disciplinary team including radiologic technologists, registered nurses, and support staff assist and support patients and physicians throughout the pre-, intra-, and post-procedure process. Patients are recovered in this area by the IRU staff and discharged same day after their procedures. The Radial Cath Lab treats an average of 25 patients/week in 1 procedure room. Hours of operation are 06:30-16:00 Mon-Thurs. They partner with the IRU Team who preps and recovers their patients.

The IRU is a sister unit to the Cath Labs. The IRU is located next to the main Cath Lab in Bldg A on the 2nd Floor next to the Prep-Recovery area and consists of 18 single patient rooms. There is also an extension of this department across campus in Bldg C conjoined with the Radial Cath Lab that allows for 5 patients. Patients come to the IRU to be prepped and then recovered from their procedure(s). Some procedures that do not require catheterization or fluoroscopy can be done in the IRU itself (Loop Recorder Insertions, Cardioversions, Trans-esophageal Echocardiogram (TEE) are just a few). The IRU also supports preps and recovers patients undergoing invasive procedures in the Radiology Department. A multi-disciplinary team including registered nurses, certified nursing assistants, pharmacy technicians, and other support staff assist patients and physicians throughout the pre-, intra-, and post-procedure process. They manage approximately 100 patients / week. Hours of operation are 05:30-21:30 Mon-Fri.

Division: Cardiac Rehab

Unit and location: First floor, Building B

Size of unit, number of beds and treatment areas: 13 staff; 0 beds/treatment areas (OP education only)

Scope of service: Specializes in monitored and supervised exercise and education helping patients who have been diagnosed with a heart condition confidently resume a normal life as quickly as possible.

Most common diagnoses: MI, CABG, PCI, Valve replacement, heart failure, LVAD, heart transplant, stable angina

Professional assets: Telemetry monitoring; blood pressures; monitored exercise program; heart disease risk factor management and education, including maintaining a healthy weight, heart-healthy eating, avoiding tobacco and environmental smoke, beginning and maintaining a personalized exercise plan, managing blood pressure, cholesterol and diabetes, psychosocial/stress assessment and counseling, opportunity to meet and share stories with other patients and improved communication with doctors/other health care providers about a patient’s progress following their cardiac event.

The Salem Hospital Anticoagulation Clinic manages the anticoagulation therapy for outpatients referred to the clinic by a licensed practitioner. The clinic serves more than 90 patients daily with disorders ranging from heart valve replacement, atrial fibrillation and deep-vein thrombosis, to those recovering from some types of stroke. RNs monitor patients during regular clinic visits to determine the best dose of therapy and work with patients to improve adherence to therapy and recommended clinic visits.

Certifications: OCN (Oncology Certified Nurse), Certified Breast Care Nurse (CBCN)

The Salem Cancer Institute Support Services team includes oncology certified nurse navigators, social workers, and dietitians providing resources and support in an outpatient setting for patients and families from point of concern throughout treatment. Nurse navigators help patients and families: identify needs, assist with problem solving, understand diagnosis and treatment options, learn what to expect from treatment, manage symptoms and side effects, navigate appointments, understand medications, find community support services, and facilitate communication between clinicians to assure continuity of care.

Certifications: OCN (Oncology Certified Nurse)

The Salem Health Clinical Research department has a variety of treatment and quality-of-life protocols available in partnership with our medical and radiation oncology providers. We have an average of 12 to 16 active trials available. We also have an affiliation with Southwest Oncology Group (SWOG) through our affiliation with OHSU that provides greater access to clinical trials and brings cutting-edge research to our community.

Certifications: OCN (Oncology Certified Nurse)

Salem Health has provided Radiation Oncology services for over 35 years. Our department is committed to offering the latest technology with the Trilogy Rapid Arc, TrueBeam stereotactic radiosurgery, Calypso, and the Varian Real-time Position Management system. We also have a dedicated treatment space for high dose rate brachytherapy. Our team includes physicians, nurses, medical assistants, therapists, dosimetrists, physicists, and support staff caring for an average of 75 patients each day.

Certifications: OCN (Oncology Certified Nurse)

Gynecologic Oncology is a joint program between Salem Health and OHSU with OHSU surgeon specialists seeing patients in clinic and performing surgery weekly in Salem. This means patients and their families can stay close to home for this specialty care. Comprehensive services include oncology certified nurses and providers, care coordination by a dedicated gyn/onc nurse, access to clinical trial networks, direct collaboration with Salem Cancer Institute support services, dedicated survivorship clinic, gynecologic cancer support group, and a community cervical cancer screening. The department sees nearly 200 patients each year.

Certifications: CWCN (Certified Wound Care Nurse), COCN (Certified Ostomy Care Nurse), CRNI (Certified Registered Nurse Infusion)

The Infusion and Wound Care Clinic is an RN-led interdisciplinary team providing physician directed care to outpatients. The patient population includes those with multiple diagnoses, infections, cancer, blood disorders and wound care not requiring admission to the hospital. Nurses also provide care for implanted intrathecal pumps. Patients are referred from primary physicians, surgeons, medical oncologists, pediatric physicians, ENT physicians, GI physicians, vascular surgeons, cardiologists, to name a few. We provide support to inpatient areas for implanted intrathecal pumps. All direct care staff are BLS-certified with two ACLS-certified RNs. Of the 23 direct care nurses, 12 are nationally certified. This clinic consists of six private wound care treatment rooms, eight private infusion treatment rooms, one outpatient ostomy room, and nine open room non-private treatment chairs. Census runs 65-to-100 patients per day.
