Scheduling tips
Here are a few helpful hints that will help streamline the check-in process when you have an imaging appointment.
Before you schedule
Schedule your exam several days before your follow-up appointment. This allows time for your doctor to receive and analyze exam results.
Please have the following information available when you call:
- Name, date of birth and Social Security number
- Emergency contact information
- Insurance card
- Name of the ordering doctor
- Type of exam
- Reason for the exam
Some of our check-in forms are available online if you prefer to fill them out ahead of time. Doing so can reduce your check-in time.
The day of your exam
Please follow all the necessary preparations for your procedure.
When you arrive you will need:
- Doctor's referral (if we did not receive it directly from your physician)
- State-issued ID or driver's license
- Insurance card
- Social Security number
- Employer's name, address and phone number
In case of a workplace injury be prepared to provide:
- Date of the injury
- Employer's insurance company name, address and phone
- Claim number
- Case manager's name
In case of an auto accident be prepared to provide:
- Date of the accident
- Name of the insured driver
- Driver's insurance company name, address and phone number
- Claim number
In a hospital setting, there are occasionally emergencies that arise that could possibly delay your appointment. It is our expectation that you should not have to wait more than 5 minutes past your scheduled appointment time.
Our commitment is to keep you informed of anything happening at the hospital that may contribute to an unexpected delay of more than 5 minutes.
