About Salem Health

Located on the fifth floor of Building B, the 46-bed clinical decision unit (CDU) is dedicated to the care of patients who present to the emergency department (ED) and require observation for up to 48 hours.

The CDU is considered to be an extension of the ED. This gives our providers and care team time to further monitor and treat patients before making the decision on discharge or admission. Diagnoses observed within this unit include electrolyte imbalance, anemia, chest pain, transient ischemia and infection. The nurses in this unit learn from a range of diagnoses, which they can add to their clinical skills toolkit.

Located on the third floor of Building A, the 30-bed general surgery unit serves pre- and post-surgical patients in the areas of gastroenterology, urology, pulmonology, ophthalmology, plastics, podiatry and gynecology. During times of overflow, the unit also cares for orthopedic and trauma surgery patients.

The general surgery unit includes Salem Health’s Bariatric Surgery Center. Nurses on this unit will receive specialty training to care for bariatric surgery patients.

Located on the fourth floor of Building B, the 24-bed inpatient rehab specialty unit provides structured interdisciplinary rehabilitation and nursing care to a diverse patient population. We treat diagnoses of traumatic brain injury, stroke, above/below knee amputation, spinal fracture, hip replacement, debility and multiple comorbidities.

The unit features a therapy gym, an apartment for practicing activities of daily living and a therapeutic courtyard. Patients receive a minimum of three hours of therapy per day in a structured environment. Rehab nurses work with patients to monitor their medical needs while they learn, use therapeutic techniques, provide education to support the healing process and collaborate with the interdisciplinary team for best patient outcomes.

Nurses take pride in the holistic approach, interdisciplinary care plan and therapeutic techniques they master to help patients. This inter-professional team also includes a physiatrist on site daily. 

• Intermediate care unit (IMCU): 30 beds on the sixth floor of Building A
• Medical surgical unit: 30 beds on the fifth floor of Building A
• General medical unit: 30 beds on the sixth floor of Building A
• Medical telemetry unit: 40 beds on the fifth floor of Building D

The intermediate care unit (IMCU) is a proud recipient of the Gold Beacon Award from the American Association of Critical-Care Nurses (AACN). Before adopting the Care in Place method, IMCU was the primary medical unit for intermediate level care. As a nurse in the IMCU, you will care for medical specialty patients, patients with a home ventilator and higher-level non-ICU cardiac patients.

All four medical units provide care to a diverse population with diagnoses of heart failure, sepsis, renal failure, respiratory failure, COVID-19 and multiple comorbidities.

Nurses get the experience of caring for patients holistically while interacting with multiple disciplines. It is a highly satisfying specialty because our nurses treat a wide range of diagnoses.

Located on the seventh floor of Building A, the 30-bed unit cares for medical surgical oncology patients. Along with specialty oncology and surgical training, nurses on this unit will obtain their chemotherapy/immunotherapy administration certification within one year of hire.

Nurses in this unit will have the opportunity to learn how to help patients and families navigate all stages of their cancer diagnosis, including end-of-life.

The services of Salem Health Cancer Institute and Salem Health’s Palliative Care team integrate with this unit to create a holistic approach to the care of oncology patients throughout the entire course of their cancer treatment. 

Located on the sixth floor of Building B, the 46-bed Orthopedic and Joint Replacement Center of Excellence specialty unit delivers care to patients with surgical and nonsurgical musculoskeletal conditions. These include general orthopedic patients with fractures and other traumatic injuries, degenerative disease, congenital disorders and infection. This unit also sees patients with total joint replacements or other musculoskeletal surgeries.

Our unit was the first in Oregon to receive a Center of Excellence Disease Specific Certification from The Joint Commission for total hip and knee replacements. We credit strong interdisciplinary teamwork for continued high-quality patient outcomes, resulting in an excellent experience for our patients.

In July of 2022 Salem Health opened a 30-bed surgical neurotrauma care unit (SNTCU) in our newly-built Building A expansion tower. Located on the fourth floor, the new SNTCU serves patients with a variety of medical needs.

Located on the fourth floor of Building A, our 30-bed intensive care unit (ICU) specializes in caring for a highly critical and diverse patient population. Patients treated on this unit include level II trauma, neurology, medical and surgical patients.

We use a multidisciplinary care delivery model with a focus on sepsis care. Growth opportunities for nurses in this unit include continuous education on renal replacement therapy (CRRT), intracranial pressure (ICP) monitoring and targeted temperature management.

Salem Hospital’s ICU was the first in Oregon to receive the coveted Beacon Award three times. It has also previously been the recipient of the Department of Health and Human Services’ award for Sustained Improvement in Elimination of Hospital-Acquired Infections.

Located on the fifth floor of Building A, our cardiovascular care unit (CVCU) features 30 beds, 18 of which are dedicated to cardiac intensive care. Our unit offers interventional cardiology and electrophysiology services. It cares for surgical cardiac, thoracic and vascular patients. We also see patients with heart failure and acute coronary syndrome.

Salem Health focuses on the growth, mentoring and retention of our nurses, which is why advanced certification is always encouraged and promoted. Our nurses train to care for the CVCU patient population through a robust training program that covers beginner to advanced cardiovascular intensive care. Trainings focus on the intra-abdominal balloon pump, continuous renal replacement therapy, targeted temperature management and ventricular assist devices.

Salem Hospital recently received its fifth designation as one of the nation’s Top 50 cardiovascular hospitals for a third year in a row. The CVCU has also received the American Association of Critical-Care Nurses (AACN) Silver Beacon Nursing Care Excellence Award three times in the past decade. 

Located on the seventh floor of Building A, the 30-bed neurotrauma care unit (NTCU) serves patients with a wide variety of brain or nervous system disorders such as multiple sclerosis, Parkinson’s disease, brain tumors, traumatic brain injuries, meningitis, encephalitis, strokes and spinal injuries.

In this unit, staff train in the National Institutes of Health Stroke Scale (NIHSS), swallow screen and spine immobilization.

The NTCU is a proud recipient of the Joint Commission Primary Stroke Center (PSC) Certification and Joint Commission Spine Center of Excellence Certification and works very closely with our Stroke Navigator.

We provide physical, emotional and educational support to all patients who require early hospitalization or are in need of high-risk antepartum care.

Pediatrics also provides pediatric outpatient care for local clinics.

Scope of Service:  Provides intraoperative care during endoscopic procedures.   Population includes adult, adolescent/young adult and geriatric in-patients and out-patients ranging from healthy to critically ill that require screening, diagnostic and therapeutic endoscopic services. RN procedures are also provided such as motility and pH tests and PEG tube checks. Anesthesia services support most procedures with RN provided moderate sedation on a limited basis.

Shift Hours: Beginning at 0630, staff arrive on staggered shifts.  Shifts generally end at 18:30. All shifts are 9 or 10-hour shifts. Call shift Hours: Call shifts are generally from 17:00 to 06:30 weekdays, and 24 hours on weekends and holidays.

Scope of service: PACU provides immediate post-anesthesia, post-surgical and post-procedural (endoscopy, interventional cardiology, interventional radiology, etc.) care to pediatric, adolescent, adult, and geriatric patients needing emergent or elective surgery/procedures as an inpatient or on an outpatient basis.

Most common diagnoses: 
• Surgical procedures range from routine appendectomies and cholecystectomies to craniotomies, video-assisted thoracotomies, and complex vascular surgeries.

Professional assets: 
• Within in the PACU, a strong critical care background is a must. 
• The post anesthesia nurse must be able to function independently and in collaboration with other nursing staff and medical providers. 
• The PACU is fast-paced environment where the bedside nurse must be able to identify trends and intervene quickly.

Shift hours and unit-based resources:
Beginning at 0700 staff arrive on staggered shifts.  Shifts are variable and include 10 and 12 hours. Call shifts: Weekend days and nights, and holidays.

Scope of service: Surgery preparation for all patients undergoing elective and emergent surgical and endoscopy procedures.  Stage 2 recovery for patients undergoing outpatient surgical procedures.  Population includes pediatric, adolescent, adult and geriatric patients.

• There is no common diagnosis.  
• As a team member you would prep and/or recover everything from an open heart to an esophogastroduodenoscopy and everything in between.

• A busy unit with quick patient turnover, requiring employees who are self-motivated, have great attention to detail, and able to deliver timely competent nursing care. 
• Frequent skills include IV starts, nerve blocks, and solid assessment skills. 
     
Shift hours and unit-based resources: 0530 - 2130, staff arrive on staggered shifts.  All shifts are 10-hour shifts. Call shifts include 15:00-21:00 weekdays, and weekend back up call from 6:00-16:30

This unit provides pre-surgery screening and evaluation of, and communication with, each surgery patient. Nurses also provide discharge phone calls following outpatient surgeries and procedures. Physician documents as well as lab and test results are collected for early chart preparation. Nurses and anesthesiologists telephone each patient for verbal counseling and information sharing, leading to complete pre-surgical case reviews prior to patient admittance. An RN is scheduled three days a week at Hope Orthopedic to provide face to face pre-surgical patient preparation.

Certifications: CRRN (Certified Rehabilitation Registered Nurse)

A 24 bed unit specializing in the physical rehabilitation of individuals with both surgical and traumatic orthopedic and neurologic conditions, strokes, and a wide variety of musculoskeletal conditions that impact an individual’s functionality and independence. The unit utilizes an interdisciplinary team model lead by 2 board certified physiatrists who are located on the unit 7 days per week. Our unit boasts a beautiful therapy garden for outdoor activities and a therapy apartment, both aimed at allowing patients the opportunity to practice skills they will need for a safe return to the community.

Scope of Service: Salem Hospital’s Rehabilitation Center is a comprehensive, 24-bed, inpatient rehabilitation facility, centrally located in the Mid-Willamette Valley. The program serves adult residents of Marion, Polk, Linn, Benton, Lincoln counties and beyond. Utilizing an interdisciplinary approach to care, our post-acute program assists individuals to maximize their independence despite their chronic disease and/or disabilities including, but not limited to:

• Strokes
• Brain injuries
• Cancer
• Spinal cord injuries
• Progressive neurological conditions
• Complex orthopedic injuries
• Loss of limb
• Trauma
• Cardiopulmonary conditions

Division: Imaging RNs

Unit and location: First floor, building A

Specialty certification needed: BLS, ACLS, PALS or ENPC may be substituted for PALS. Telemetry monitoring and demonstrated proficiency in cardiac rhythm interpretation. Conscious sedation competency mandatory. Nurses should have critical care background. Optional certification in certified radiology nursing (CRN).

Size of unit, number of beds and treatment areas: Four patient bays located in Imaging Staging for short term monitoring or minor procedures. There are no licensed beds in imaging, however there are 25 patient exam rooms in imaging that consist of the following types: X-ray, Mammography, Ultrasound (US), Computerize Tomography (CT), Magnetic Resonance Imaging (MRI), Nuclear Medicine, and Positron Emission Tomography (PET). Of these modalities, there are about 7 rooms dedicated to invasive procedures.

Scope of service: The Imaging department at Salem Health provides care for patients in need of Imaging and Diagnostic procedures. This includes inpatients and outpatients as referred by physicians.  Patient population ranges from pediatric to geriatric, but is predominately adult and geriatric patients. Imaging RN’s should have a critical care background and be certified in BLS, ACLS, PALS, and maintain conscious sedation competency. Nursing care encompasses minimally and moderately invasive procedures, which may or may not require conscious sedation. In addition, conscious sedation may be utilized for non-invasive imaging scans for patients with claustrophobia or pain control issues. We also support the floors for some procedures requiring RN sedation.

Most common diagnoses: ESLD, cirrhosis, Hepatitis A/B/C, pulmonary disease, heart failure, renal disease, cancer, r/o cancer, cardiac arrhythmia, r/o MI, drain placement.

Professional assets: Imaging RNs work hand-in-hand with a variety of professionals including radiologists, interventional radiologists, physician assistants, ultrasound technologists, CT technologists, MRI technologists, NM technologists, X-ray technologists, mammography technologists and telemetry trained unit clerks. Imaging RNs also provide back-up care for cardiologists.

Shift hours and unit-based resources: Imaging RNs staff between the hours of 0700 and 1930, Monday thru Friday. Weekend coverage is between the hours of 0800 and 1830. An imaging RN is on call during holidays from 0800-1630.

Collaborative resources: Imaging doctors, physician assistants, lead imaging technologists, and imaging managers. Imaging RNs can also call on house resources such as rapid response team, assigned house supervisor, patient transportation, and interpretative services.

We are a 7 suite Cath Lab that consists of 5 suites in our main lab, 1 outpatient suite in the COM and a hybrid suite in the OR. Our team consists of RNs and Radiologic Technologists that assist in performing state of the art procedures including interventional cardiology, interventional radiology and Electrophysiology. Our patient population consists of persons of all ages that are in need of diagnostic and/or interventional imaging. We proudly boast an average time of 60 minutes from arrival to balloon angioplasty (door to balloon time) for patients with Acute Myocardial Infarctions. The gold standard nationwide is less than 90 minutes. We, as a department, are constantly looking for areas of improvement in our practice through our Unit Council and input from staff members. All staff members are ACLS/BLS trained.

Outpatient Cath Lab:  This is a 1 room cath lab that consists of a team of RNs, Radiologic Technologists and Cardiologists that perform state of the art radial cath lab procedures.

The Radial Cath Lab is an extension of Cath Lab services, located across campus from the main Cath Lab, in Bldg C, 3rd Floor. The Radial Cath Lab is focused exclusively on the outpatient population where catheterization is suited for the radial artery approach. This department supports freely mobile adults with cardiovascular disease referred by primary care physicians and cardiologists. Patient needs include cardiac diagnosis and intervention by fluoroscopy along with continuous cardiac monitoring, procedural sedation, and occasionally other interventions. A multi-disciplinary team including radiologic technologists, registered nurses, and support staff assist and support patients and physicians throughout the pre-, intra-, and post-procedure process. Patients are recovered in this area by the IRU staff and discharged same day after their procedures. The Radial Cath Lab treats an average of 25 patients/week in 1 procedure room. Hours of operation are 06:30-16:00 Mon-Thurs. They partner with the IRU Team who preps and recovers their patients.

The IRU is a sister unit to the Cath Labs. The IRU is located next to the main Cath Lab in Bldg A on the 2nd Floor next to the Prep-Recovery area and consists of 18 single patient rooms. There is also an extension of this department across campus in Bldg C conjoined with the Radial Cath Lab that allows for 5 patients. Patients come to the IRU to be prepped and then recovered from their procedure(s). Some procedures that do not require catheterization or fluoroscopy can be done in the IRU itself (Loop Recorder Insertions, Cardioversions, Trans-esophageal Echocardiogram (TEE) are just a few). The IRU also supports preps and recovers patients undergoing invasive procedures in the Radiology Department. A multi-disciplinary team including registered nurses, certified nursing assistants, pharmacy technicians, and other support staff assist patients and physicians throughout the pre-, intra-, and post-procedure process. They manage approximately 100 patients / week. Hours of operation are 05:30-21:30 Mon-Fri.

Scope of service: Specializes in monitored and supervised exercise and education helping patients who have been diagnosed with a heart condition confidently resume a normal life as quickly as possible.

Most common diagnoses: MI, CABG, PCI, Valve replacement, heart failure, LVAD, heart transplant, stable angina

Professional assets: Telemetry monitoring; blood pressures; monitored exercise program; heart disease risk factor management and education, including maintaining a healthy weight, heart-healthy eating, avoiding tobacco and environmental smoke, beginning and maintaining a personalized exercise plan, managing blood pressure, cholesterol and diabetes, psychosocial/stress assessment and counseling, opportunity to meet and share stories with other patients and improved communication with doctors/other health care providers about a patient’s progress following their cardiac event.

The Salem Hospital Anticoagulation Clinic manages the anticoagulation therapy for outpatients referred to the clinic by a licensed practitioner. The clinic serves more than 90 patients daily with disorders ranging from heart valve replacement, atrial fibrillation and deep-vein thrombosis, to those recovering from some types of stroke. RNs monitor patients during regular clinic visits to determine the best dose of therapy and work with patients to improve adherence to therapy and recommended clinic visits.

The Salem Cancer Institute Support Services team includes oncology certified nurse navigators, social workers, and dietitians providing resources and support in an outpatient setting for patients and families from point of concern throughout treatment. Nurse navigators help patients and families: identify needs, assist with problem solving, understand diagnosis and treatment options, learn what to expect from treatment, manage symptoms and side effects, navigate appointments, understand medications, find community support services, and facilitate communication between clinicians to assure continuity of care.

The Salem Health Clinical Research department has a variety of treatment and quality-of-life protocols available in partnership with our medical and radiation oncology providers. We are a member of Southwest Oncology Group (SWOG) that provides greater access to clinical trials and brings cutting-edge research to our community. We also partner with Oregon Oncology Specialists to provide provides the latest treatment options through clinical and pharmaceutical trials.

 

Scope of Service: Specializes in the administration of radiation treatments to cancer patients. Nurses in this outpatient clinic follow evidence based oncology guidelines to manage side effects and support patient care. Radiation Oncology is part of Oncology Services, which is accredited by the Commission on Cancer and the National Accreditation Program for Breast Centers. The Cancer Support Services team works collaboratively with the Radiation Oncology team to create a holistic approach to the care of oncology patients throughout their journey through cancer treatment.

Shifts hours: Monday through Friday 7:30 a.m. to 4:30 p.m.

Scope of Service: Oncology surgical clinic specializing in gynecological and breast cancers. Nurses in this outpatient clinic follow evidence based oncology guidelines to assess, triage and provide care for oncology patients. Gynecological Oncology is part of Oncology Services, which is accredited by the Commission on Cancer and the National Accreditation Program for Breast Centers. The Cancer Support Services team works collaboratively with the Gynecological Oncology team to create a holistic approach to the care of oncology patients throughout their journey through cancer treatment.

Infusion Clinic

The Infusion Clinic is an RN-led interdisciplinary team providing physician directed care in the outpatient setting. The patient population includes, but is not limited to infections, cancers, blood disorders, and auto-immune disease.  Patients are referred from primary physicians, infectious disease providers, surgeons, medical/radiation and gynecological oncologists, pediatric physicians, ENT physicians, GI physicians, vascular surgeons, and cardiologists to name a few. All direct care staff are BLS certified. The department also has a high number of nationally certified oncology and infusion nurses. The clinic consists of some private infusion treatment rooms and an open non-private treatment room.

Advanced Wound Care

Advanced Wound Care is a physician-led interdisciplinary team providing direct care to patients with chronic, non-healing, and complex wounds in the outpatient setting.  Treatments include Negative Pressure Wound Therapy, application of biological skin substitute, casting, biopsy, and wound debridement. Patients are referred from primary physicians, surgeons, medical/radiation oncologists, and vascular surgeons. All direct care staff are BLS certified. The department also has nationally certified wound care nurses. The clinic consists of private wound care treatment rooms.

Wound & Ostomy

The Certified Wound and Ostomy Nurses provide care to patients with complex wounds in the hospital setting. These nurses serve as direct care, consultants, and educators for wound assessment and wound management. This team also cares for ostomy patients while in the hospital and provides post op care and management in an outpatient clinic.
